Changes Form	Use to send in changes to Team information
Transfer Form	BC Soccer form (Send to FVSL ONLY)
Youth-to-Adult Permit	BC Soccer form (Send to FVSL ONLY)
Youth Application to Transfer to an Adult Team	BC Soccer form (Send to FVSL ONLY)
Youth Reinstatement	(Youth player registered to an adult team being reinstated to youth status)(Send to FVSL first)
Referee Performance Form	This is a form you can fill in online that is automatically emailed to the FVSL.
Accident Claim Form	Have you suffered a serious injury while playing in the FVSL?
Move/Merge Request	Teams requesting to move clubs should use this form. Send to FVSL.
